Children must be five years old on or before August 20, 2026, to be eligible to enroll. Families will complete the registration process online through FinalForms at: urbana-oh.finalforms.com.

Parents who are new to the district will need to create an account and register their student, while families with a current preschool student should log in to their existing FinalForms account and click “Update Forms” to move their child to Kindergarten. All forms must be completed and submitted before scheduling a screening appointment. Please click HERE for a full set of registration directions.

After completing the online forms, call Mrs. Robin Miller at the Board Office: 937-653-1481, between 8:00 am and 3:00 pm to schedule a Kindergarten screening appointment for Wednesday, August 5, which will be held at the Urbana Elementary School building at 1673 US Highway 68, South, Urbana.
